Nothing is provided for the theme but the gift shop usually has various, expensive, theme items if you don't want to bring stuff.
Note that *everybody* doesn't do anything. In other words, some people go all in, some pay light homage to the theme, and some do their own thing. You will not stand out regardless of what you do - unless you, uhh, "stand out" (so to speak...). Think of the theme participation this way: If you didn't know the theme and wandered into the disco, you could probably, maybe figure out what it was. Like, "Hmm, my guess is it's some sort of white theme. Oh, and look, there are some people wearing masks. Masquerade??" But you wouldn't be 100% sure because there will be people with only a white shirt, no masks, or whatever.
The themes are fun to plan for and give you something that keeps the focus on the upcoming trip for however long that is. But, don't get worked up worrying about whether you have the right stuff or not - or if you don't "feel it" for one or more themes. The only way you will feel out of place is if you (and by "you" I am primarily talking about the wife) wear something you would wear to a restaurant back home. If you don't push the envelope at least a little bit, you will probably wish you had.

Update: I should probably add that people who *do* go all-in on the themes, do get accolades for their effort from the people there so if that *is* your thing, go for it!
