Erotic massage

Planning to go for our second trip, in August. On our first visit, we enjoyed Claudias' massage so much that it was a main requisite for this trip, and I've already made a reservation with her. I have also gotten a tentative go-ahead (from my wife) to book the erotic massage.... But after much reading she asked me to get a copy of the form that one fills out prior to getting the massage (where, among other things, we'd let them know what we are comfortable with). Does anybody know where to get a copy of that form?
