I understand that Desire now charges $5 for room service.

Is this a flat fee per order?
Is it added onto your guest tab or paid up front?
Do you still tip?

I'm not sure I understand the concept behind this fee.

Yes , you are correct. There is a $5 fee that is charged to your account (to be settled upon check out) for each time you use room service. It is a flat fee per use.

We always tip the waiter extra if they do a great job ( and they Always have !) . They usually will come in, set it all up, remove wraps and coverings, make sure everything is to your satisfaction, and come back an hour or so later to collect the dishes outside your door (so the bugs dont swarm to them)

The concept we are told is that several years ago, guests were being abusive of the system and ordering one thing, and then 30 minutes later another, and so on. Don't know if its true but that is what we were told.
